Somewhere between 40 and 55 many men notice it: training delivers less than it used to, the belly stays despite discipline, sleep is restless, and the appetite for a lot of things fades. The usual line is "that's just age". It may be true. It may just as well be chronic stress, too much oestradiol, or adrenal glands that deliver less reserve after years of running flat out. From the outside it all looks the same.
A profile separates these cases. Testosterone is the starting point, but only in relation to oestradiol does it become clear whether too much is being converted in the oestrogen direction, which often goes hand in hand with belly fat. DHEA shows whether the adrenal glands still supply the precursors; it peaks around 30 and declines steadily after that. Cortisol often explains why testosterone drops: both are built from the same raw-material pool, and chronic stress pulls production to its side. The three morning samples capture precisely the moment when these values are most meaningful.

1. Collect the samples: Fasting in the morning between 7 and 9 am, give saliva through the straw into the three numbered tubes. Intervals, preparation and what to do about hormone preparations are in the instructions for use (PDF, German).
2. Send them in: Place the tubes in the protective container, add the completed sample form, and post everything in the prepaid return envelope.

Why saliva rather than blood?
In blood, more than 95 percent of testosterone is bound to SHBG and albumin and therefore unavailable to cells. Total testosterone from a blood draw says little about how much is actually working, especially since SHBG rises with age and pushes the free share down further. Only the free fraction diffuses into saliva. A blood value within range and a low saliva value are therefore not necessarily a contradiction; they measure different things. Add the practical side: saliva can be collected at home, without a needle and at the right time of day, which is decisive for testosterone and cortisol with their morning peak.
How does this differ from the Hormone Profile Men without "Plus"?
The basic profile measures testosterone and DHEA. The Plus version adds oestradiol, progesterone and cortisol. That pays off when belly fat, sleep problems or chronic stress are part of the picture, because then the cause often lies not with testosterone alone but in its conversion to oestradiol or in the stress axis. If you only want to know whether your testosterone is fine, the basic profile will do. For a first overall picture, the Plus profile is the more complete choice.

Who is the test for?+
Men who want to know where their hormones currently stand: with fading drive, libido or strength, sleep problems, weight gain around the middle or persistent exhaustion, but also as a baseline from 40 onwards or as a follow-up when you change something about training, sleep or stress and want to see whether it shows in the values.

Why does it have to be in the morning?+
Testosterone and cortisol follow a daily rhythm with their highest values in the first hours after waking. In the afternoon testosterone can be a third lower without anything having changed hormonally. To keep the values comparable, the samples belong in the window between 7 and 9 am, before coffee, breakfast and brushing your teeth.

What do I need to keep in mind before collecting saliva?+
Collect fasting, so no eating, drinking or brushing your teeth beforehand. How to proceed with hormone preparations, gels or supplements and at what intervals the three tubes are filled is explained in the instructions for use linked above (German). Read them the evening before and the morning runs smoothly.

How do I receive my report?+
Through medivere's online results portal, usually 4 to 7 working days after the lab receives your samples. Why is the test sold to Austria and Germany only?+
The samples have to reach the laboratory in Mainz promptly using the enclosed return envelope. The kit is designed for that postal route from Austria and Germany. From other countries we can offer neither shipping nor analysis.

My testosterone is low. What now?+
Take the report to your doctor or an endocrinology or urology practice. They will clarify whether further values such as LH, FSH, SHBG or thyroid function belong in the picture and which steps suit you. Adding hormones on your own is not a good idea, not least because external testosterone suppresses your own production. With five values in hand, that conversation becomes far more concrete.
